1. A 36 year old office worker presents to A&E with severe right loin pain and haematuria.  An abdominal ultrasound reveals hepatomegaly and bilaterally enlarged kidneys. He has a family history of hypertension and his grandfather died from a stroke.

 

 

2. A primary school child who previously presented with her father to her GP with impetigo, presents again 4 weeks later with a swollen face and blood in her urine. Red blood cell casts are seen on microscopy.

 

 

3. A 6 year old child with worsening deafness is found to have microscopic haematuria and proteinuria on a routine check-up.  Electron microscopy of a renal biopsy shows diffuse irregular thickening of the glomerular basement membrane with a basket weave appearance.

 

 

4. Urine dipstick shows that a 55 year old woman is suffering from recurrent episodes of haematuria and proteinuria.  She is otherwise well.  She has a renal biopsy which under florescence microscopy shows IgA deposition and proliferation of the mesangium. 

 

 

5. A fit and healthy 81 year old lady is found trapped beneath a collapsed table in her garden shed.  Her renal function is poor on admission with an elevated creatinine and urea, poor urine output and hyperkalaemia.
