1. A 40 YO gentleman comes in to the hospital routinely for venesection. On this occasion he also complains of S.O.B. on exertion, and a plain-film CXR reveals an enlarged cardiac shadow.
2. A 45 YO gentleman has recently moved to the UK from the tropics and comes into hospital with S.O.B. at night. His bloods show a raised eosinophil count for the past year
3. A 36 YO lady with known SLE, incidentally found to have a low-grade pan-systolic murmur, is referred to cardiology. Echo shows some small vegetations, and you see focal necrosis on H&E staining.
4. A 50 YO lady presents with S.O.B. and chest pain on exertion. O/E you hear a mid-systolic click followed by a late systolic murmur, growing louder when the pt. stands up.
5. A mother of a 4 month old baby comes into hospital with a persistently distended abdomen and oedematous legs, without any previous cardiac history.
