1. A 54 year old gentleman was referred from his GP and he presents with a history of recent weight loss, fatigue and is febrile. On examination of his hands, you find small red linear red lesions.
2. A 15 year old girl returns to her GP after recently been treated for a sore throat, this time complaining of fever, being short of breath and general malaise. On questioning, you find that these symptoms have been associated with a series of painful elbow and knee joints.
3. A 57 year old lady presents complaining of palpitations, shortness of breath whilst sitting and relieved whilst supine, and involuntary weight loss. An echocardiogram displays a polypoid mass attached to the septal wall in the left atrium.
4. A 42 year old male presents to A&E with sharp chest pain that is relieved by sitting forward. On examination you find a pericardial rub at the left sternal edge.
5. A 21 year old semi professional footballer suddenly collapsed during a match and was pronounced dead on the scene. There is no significant past medical history apart from a previous episode of syncope during a training session.
