1. A 67 year old woman presents to A&E after collapsing. Her daughter mentions that her mother eyes always looked down and out and her pupils always looked big.

 

 

2. A 72 year old man with a history of poorly controlled hypertension presents to the clinic. His wife is concerned because he has had several episodes of weakness that have resolved, as well as temporary blindness and facial droop.

 

 

3. A young woman of reproductive age (who denies being pregnant) presents to A&E after a week of worsening headaches. While there she develops right-sided upper limb weakness and difficulty speaking.

 

 

4. A 65 year old man presents to A&E with vomiting and nystagmus. A neurological examination reveals right sided ataxia, left sided loss of pain and temperature peripherally, right sided loss of pain and temperature on the face, right sided Horner’s Syndrome and dysphagia.

 

 

5. A medical student playing Ultimate Frisbee, dives for a catch and hits the right side of her head on a strangely located brick. She makes the catch and wins the game, saying she is fine. Later at home she collapses after retelling her story to her boyfriend over Skype.
