For each scenario below, choose the most appropriate answer from the list above. Each option may be used once, more than once or not at all.

  1. A 48 year old woman presents to her GP with a recent and sudden weight gain. She has gained 6kg in the past 6 months not associated to her diet. She was recently diagnosed with sarcoidosis and is on treatment for this. On examination, she appears to have purple lines around her abdomen. She also notes that she has noticed that she bruises easily and has recently developed hair growth on her face.

2.   A 56 year old man presents to clinic complaining of back pain. He has a 4 year history of well-controlled type II diabetes mellitus. The patient has brought in a photograph of himself taken 8 years ago and points out that his facial features have changed undesirably and also that his shoes feel too tight.

3.   An anxious 25 year old woman attends the family planning clinic complaining of an irregular menstrual cycle. Her partner and her are trying to get pregnant and want advice on this matter. The patient notes that her periods have only become irregular the past few months and she has never taken OCP. On examination, she is very fidgety and has an abnormal ECG. She adds that she has been eating a lot more recently but has surprisingly lost weight.

  1. A 25 year old woman is brought into A&E by her mother as she had a seizure in her home the hour before. She appears to be very ill, weak and confused. Her mother noted that she had gone through a recent break-up with her partner and has since been obsessing with her ‘puffy’ features due to her long-term medications for her medical condition as the cause for her break-up.

  1. A 40 year old man presents to his GP complaining of excessive thirst and frequent urination. He claims to have to drink more than 4 litres of water a day as he is always thirsty. A fluid deprivation test shows an urine osmolality of 700mOsm/L. His normal urine osmolality is 280mOsm/L. After desmopressin administration, his urine osmolality is 1200mOsm/L.
