1. A 62-year-old professional spoon player from Canada has recently undergone surgical resection for hepatocellular carcinoma. Before the operation he was given a prophylactic cephalosporin. He presents with a fever, offensive smelling diarrhoea and severe abdominal pain. He is anaemic and has an elevated white blood cell count and low serum albumin.
2. A 35-year-old Irish business-woman, who owns a specialist mail order delivery company, presents with a long standing history of profuse watery diarrhoea. Colonoscopy appears normal but colonic biopsies reveal a high number of inflammatory cells.
3. A 31-year-old steer wrestler who has a talent for speaking backwards presents with a history of diarrhoea and weight loss. He admits to noticing blood and mucus in his stool. On examination he has clubbing and numerous mouth ulcers. Colonoscopy reveals a continuous area of inflammation.
4. A 24-year-old medical student, returning from an elective down under, presents with abdominal pain, diarrhoea and weight loss. He also admits to having an itchy anus and occasional faecal incontinence. Colonoscopy reveals patchy inflammation.
5. A 15-year-old female who recently had what she said was the worst takeaway ever presents with an acute history of diarrhoea and vomiting. She admits to noticing blood in her stool.
