1. 1. 50 year old poorly controlled diabetic with history of leg ulcers presents with extremely painful swollen shin. Examination shows crepitus on palpation. Culture of aspirate shows presence of double beta haemolysis.
2. 2. 42 year old South African man presents with fever, weight loss and diarrhoea. CXR shows lower lobe intersititial infiltrates. CD4 count is 180.
3. 3. A 25 year old woman presents with painful swollen elbows and wrists, and a papular rash. Further questioning reveals a history of vaginal discharge. Joint aspirate shows gram-negative intracellular diplococci.
4. 4. 5 year old presents with fever and a painful swollen thigh. Blood culture shows clusters of gram positive cocci.
