1. A 58 year old lady had persistent fever for 3 weeks, she felt generally ill and noted brown coloured urine and some painful discouloured “lumps” in her hands. She had been an IV drug user in the past. After several investigations and after two positive cultures she was treated with a combination of ampicillin and gentamicin.

 

2. A keen 24 year old triathlete on a training camp in the Rocky Mountains, had a fever for 3 weeks and now has a severe headache and had been sick on numerous occasions, just before presenting to hospital a week ago he noticed a rash forming on his ankles which the doctor described as macular, it has since spread more centrally. He noted having been “bitten” by small insects a few weeks before.

 

3. A 25 year old that has recently moved from South Africa presented a week ago feeling tired and drained, she had a fever and particularly noticed this at night were she sweated a lot. She is HIV positive.

 

4. Johnny, an 18 year old has had a fever, felt cold and had a headache for two weeks and has been in hospital for over week with no identifiable cause found. He has a rash that resembles a dart board and recently notes muscle pain and a swollen knee. His only recent holiday was to the new forest 3 weeks ago.

 

5. A 30 year old male presents to you with a swollen and painful right elbow with grossly reduced range of movement. He has a heart rate of 50 and a rash.
