For each scenario below, choose the most appropriate treatment from the list above. Each option may be used once, more than once or not at all.

 

1.  A 67 year old woman presents to A & E with a productive cough, fatigue and high fevers. While taking a history it appears that she is extremely confused. Examination reveals bilateral crepitations, a blood pressure of 90/50 and a respiratory rate of 40. A chest x-ray reveals consolidation and her urea on routine bloods is found to be 9 mmol/l.

 

 

2.  A 48 year old man presents to A & E with shortness of breath on exertion. His examination findings are normal and there are no obvious changes on chest x-ray. His oxygen saturations fall to 75% on standing. He has just returned from a trip to Thailand.

 

 

3.  A 5 year old girl presents to A & E with a history of cough and a runny nose. She has a dispersed erythematous macular blanching rash on her trunk. Her chest is clear on auscultation, but her right tympanic membrane is inflamed as are her tonsils.

 

 

4.  A 62 year old ex-smoker on CPAP begins to lower his oxygen saturations and is experiencing haemoptysis. A chest x-ray shows right lower lobe consolidation. He has been in hospital for 4 weeks.

 

 

5.  A 3 week old boy presents to A & E with signs of respiratory distress. His x-ray shows ground glass infiltrates. Social services inform you that his mother is an IVDU.
