1. A 18 year old girl presents to her GP with an erythematous, painful rash over the shins. She complains of swollen glands in neck and a persistent fever, for which her blood film confirms a positive heterophil antibody test.

 

2. A 65 year man presents to A&E with multiple large tense blisters on his limbs and trunk. Blisters vary in size. Biopsy shows separation of epidermis and dermis, with eosinophilic infiltration.

 

3. A 15 year old girl presents with a diffuse, itchy blistering rash on the extensor surfaces of her limbs and buttocks. On routine questioning, she notes that her stools have been difficult to flush away and has lost 4kg in the last 2 months. Subsequent skin biopsy from an unaffected area shows subepidermal IgA deposition.

 

4.  A 32 year old ex-smoker with known episodic bloody diarrhoea presents to his GP with a 2 week history of an ulcerating lesion over his left shin. Previous colonoscopy reports multiple superficial ulcers seen in the descending colon.

 

5. A 72 year old woman presents to A&E with extensive blisters, some of which have become denuded, showing a raw and erythematous skin beneath. Subsequent skin biopsy shows acantholysis in epidermal layer.
