1. A 50 year old man presents with excruciating pain in his 1st metatarsophalangeal joint. The joint looks red and inflamed. An aspirate reveals crystals that appear blue when perpendicular to a red compensator.
2. An 18 year old man has a history of self-mutilation. On further investigation the doctor mentions he has high levels of uric acid in his urine.
3. A 40 year old factory worker has been involved in making jet engines and ammunition for the army for the last 20 years. He presents with a swollen, red and painful ankle. He mentions that he has been feeling depressed recently. You carry out a full blood count and notice that he is anaemic.
4. A 70 year old woman presents with acute swelling of her wrist. The joint is extremely painful. On aspirate you find crystals that appear blue when parallel to a red compensator.
5. A 21 year old man recently back from Spain, has developed a painful and inflamed knee. He comments that his father recently had a swollen and red toe. On further investigation you find a raised CRP and ESR.
