1. An 18 year old student returns from their gap year where they worked in a school in Nepal. A month after returning they go to their doctor complaining of lethargy and malaise. A week later they develop jaundice. They recovered without treatment before the start of university 2 months later.
2. A 16 year old boy presents to his GP with a 3 week history of malaise and fatigue. In the last week he has noticed jaundice and fever. A blood count and film revealed a lymphocytosis with atypical lymphocytes. Paul-Bunnell test is positive.
3. A 30 year old Thai lady with a 5 year history of liver cirrhosis presents to her GP with weight loss. Her mother is known to have had active infection with a dsDNA virus at the time of her birth in Thailand. Hepatocellular carcinoma is found.
4. A pregnant Indian lady returns to her family pig farm in India for a wedding. On her return she became jaundiced, developed ascites and abdominal pain. She was admitted to hospital but her condition worsened. She became confused and increasingly drowsy and despite supportive treatment died 3 weeks after admission.
5. A 36 year old man gives blood. He is rung the next day by the Blood Service and told that antibodies to a ssRNA virus had been found. He claims that he has never been jaundiced but admits to using intravenous drugs in his teens. Scan of his liver reveals early cirrhotic changes.
