1. A 42-year-old lady presents with extreme fatigue and dark-coloured urine. She also describes some nasal stuffiness and recurrent nosebleeds. Her urine dip shows RBC +++ and serum U&E shows elevated creatinine and urea, as well as a dangerously high potassium. She is admitted urgently to the renal unit at Hammersmith Hospital.
2. A young woman has positive serum ANA and anti-dsDNA antibodies. A skin biopsy is taken which shows her skin contains deposits of IgG and complement.
3. A Japanese woman presents at her GP, who upon routine examination, cannot locate radial or brachial pulses. The patient reports symptoms of fatigue, night sweats, dizziness and visual disturbances.
4. A young Armenian man presents to A&E with acute severe abdominal pain. CRP and WBC are both raised. He stays overnight, sustaining an unremitting fever of 39.5°C.
5. A nervous, middle-aged woman presents to her GP complaining of palpitations. As she complains, her GP notices a tremulous grip and protruding eyes.
