1. A 23-year old Afro-Caribbean woman presents to her GP with malaise and complaining of a mild rash on her face. Routine investigations reveal pancytopenia and mild renal impairment.
2. A 48-year old man presents with increasing pain and weakness of his upper limbs and neck muscles. On examination you note a rash around his eyes and the extensor surfaces of his elbows. Investigations reveal a raised Creatine Kinase and he tests positive for Anti-Jo-1 antibodies.
3. A 39-year old woman presents with generalised muscle pain and intermittent swelling of both hands and an occasional loss of colour in her left middle finger. On examination, you observe sclerodactyly, mild alopecia and conjunctival pallor.
4. A 36-year old man presents to A&E with a blood pressure of 190/120 and frank haematuria. Renal angiography reveals multiple renal aneurysms. C-ANCA and p-ANCA are both negative.
5. A 2-year old girl is brought to hospital by her mother who tells you her daughters hands have become very red and swollen and she has had a fever for 2 days that has not responded to paracetemol. On examination you not bilateral conjunctivitis, erythema and oedema of the lips and a ‘strawberry tongue’.
