For each scenario below, choose the most appropriate answer from the list above. Each option may be used once, more than once or not at all.

1.  This is a chronic infection of the cutaneous or subcutaneous tissue and the adjacent lymphatics. There are nodular lesions which may suppurate and ulcerate. This may also spread to articular surfaces, bone and muscle. It is found worldwide.

2. This is a chronic granulomatous disease (CGD) of the nasal submucosa. The clinical sequelae depend on the causative organism, with polyps or palpable masses appearing with Conidiobolus coronatus, or CGD that is restricted to the limbs, chest, back and buttocks with Basidiobolus ranarum. This is seen mainly in the tropics.

3. This infection can manifest as a multisystem disease process because of its invasive nature. It can cause endocarditis, pericarditis, suppurative phlebitis, meningitis, arthritis and peritonitis. It is responsible for a large proportion of mortality due to nosocomial infection.

4. This is an acute, rapidly developing infection of the cutaneous and subcutaneous tissue, causing patches of necrosis. It is often fatal in debilitated or diabetic patients. It is found worldwide.

5. This is a chronic subepidermal infection. It can form keloidal, verrucoid nodular lesions as well as crusty plaques, and there

is no systemic spread. It is restricted to the Amazon Valley in Brazil
