1. persistent monocytosis of >1 x 10 /L with splenomegaly and a skin rash

 

 

2. findings on examination of bone marrow include: <5% blasts, megakaryocytes with hypolobulated nuclei. The cytogenetic abnormality has a good prognosis. This patient was then started on lenalidomide
