For each scenario below, choose the most appropriate answer from the list above. Each option may be used once, more than once or not at all.
1. A 3 year old boy presents with a high fever of 3 days. Examination shows a very irritable child with mild conjunctivitis, a rash, cervical lymph nodes and cracked red lips. The consultant is worried and sends the child for further investigations including echocardiography before starting to treat the child with intravenous immunoglobulin.
2. A 7 year old presents with fever, headache and irritability. On examination the child is found to have a positive Kernig sign. The lumbar puncture sample appears to be turbid.
3. A final year medical student presents with her 11 year old boy. The child complains of fever, anorexia and weight loss. Chest X-ray shows changes. The mother and child have just returned home from visiting their family in Gujarat.
4. A homeopath brings her 4 year old boy to see the doctor. The child has had a high fever for the previous week but seems to have settled following an intensive course of highly potent 40C homeopathic medication. However, the cough which started with the fever is still present. On examination the child is found to have a facial rash and Koplik’s spots.
5. An ICSM netball player brings her 3 year old twins into see their GP following an itchy rash. Both Twin A and Twin B have a fever as well as typical vesicular rash. The rash appears to be formed of crops of papules, vesicles and pustules.
