1. Whilst on GP placement a 16 year old boy presents with pain in his knee that has been ongoing for the last 6 months. It is worse at night. The GP sends him for an X-ray and the report mentions ‘Codman’s Triangle.’ The GP asks you if you have ever heard of this and what may be causing the pain.
2. Mrs X, an 80 year old smoker, is brought into your clinic reluctantly by her daughter complaining that she has been coughing up blood and has difficulty breathing for years. Her daughter complains she has become so frail Mrs X has had to move in with her and her husband and the constant coughing is keeping them up at night. Mrs X says she has back pain which is worse at night and with rest.
3. A mother brings her 4 year old child so see you because she thinks her sons eyes are ‘popping out of his head’ and no other children at playschool have eyes like that. On examination you find he has a dysconjugate gaze. Histopathology shows small round cells that on immunostaining are positive for desmin.
4. A 17 year old girl presents with pelvic pain. As part of her work up she has genetic testing and is found to have a chromosomal translocation 11:22 (EWS/Fli1). On biopsy a small blue cell tumour is found.
5. A mother brings her daughter to your paediatric clinic because her son keeps getting himself into sticky situations and has broken his arm 4 times in the last year. Before you jump to conclusions you examine him and notice his sclera are blue.
