1.   An 87 year old woman presents with back pain. On examination there are signs of anaemia and a large breast lump.

 

2.   A 16 year old girl presents with sudden onset of fever, malaise and severe pain in her arm. X-ray shows a lytic lesion with an ‘onion skin’ type periosteal appearance in the humerus.  

 

3.   A 14 year old boy presents to A&E after he fractured his femur playing table tennis. Previously he had complained of knee pain and swelling. X-ray findings include a lytic metaphyseal lesion of the distal femur and you identify Codman’s triangle. 

 

4.   A 40 year old woman presents with a history of gradual swelling and worsening pain of her left knee. X-ray shows a lytic lesion of the distal femoral epiphysis. A biopsy is performed and histology shows osteoclasts on a background of spindle cells.
