1. A 60 year old actor telephones his transplant team 6 weeks after a renal transplant complaining of fever and shortness of breath. He has also had some aches in his muscles and joints and feels very weak. His xray shows interstitial infiltrates. WCC is 2.2 x 10^9 / L
2. A 16 year old boy complains of a 2 week history of lethargy, malaise and fever with myalgia. In the last 3 days he has developed a productive cough. His chest x-ray shows cavitating lesions with fluid levels in the right middle zone. Sputum culture shows gram positive cocci.
3. A 39 year old midwife presents with a productive cough that has not resolved for three months. Her sputum occasionally contains blood. She feels “generally run-down,” and has lost some weight because she has just returned from a very hectic year working with Medicines Sans Frontieres.
4. A seven year old girl is brought in by her mother who says she has had a cold for two weeks and her coughing is getting worse. You are startled by the loud inspiratory noise she makes after coughing.
5. A woman presents with fever, cough and chest pain. Past medical history includes severe asthma and SLE. The lab reports galactomannan is present in her serum sample.
