1. A 24 year old woman presents with a 3 day history of fever, headache, myalgia and dry cough. Chest x-ray shows patchy consolidation of the left lower lobe. She recovers fully following a course of antibiotics, but returns to A+E 3 weeks later with worsening bilateral leg weakness.

 

2. An 80 year old man presents with bilateral cavitating bronchopneumonia. Sputum culture shows gram positive, beta-haemolytic clusters.

 

3. A 40 year old man presents with a 2 day history of headache, fever, diarrhoea and vomiting. Further questioning reveals that he keeps two parrots at home.

 

4. A 50 year old woman with known mitral valve disease presents to her GP complaining of feeling feverish and generally unwell over the past few weeks. Auscultation of the heart reveals a new murmur. Blood culture shows Gram positive organisms that produce a green colouration on blood agar plates.

 

5. A 17 year old man presents to his GP complaining of a red sore area on his face which has been present for the past week. He suffers from acne, but his benzoyl peroxide cream has produced no improvement. He says that the sore initially produced a lot of serous fluid but has now crusted over
