1. A 70 year old gentleman with a significant smoking history presents to his GP with weight loss and progressive shortness of breath. O/E he is underweight, barrel chested and dyspnoeic. He is breathing through pursed lips. A diagnosis is made following spirometry.

 

 

2. A 65 year old retired secretary presents to her GP with progressive shortness of breath on exertion. O/E she is clubbed. She has never smoked and is otherwise well. Lung biopsy later shows a variable loss of cellularity and patchy deposition of collagen.

 

 

3. A 25 year old woman is admitted with a suspected pulmonary embolism. Several days later, routine bloods show a creatinine of 400μmol/L. She admits to seeing some blood in her urine the previous day, and that she has not felt the need to urinate at all since last night.

 

 

4. A 45 year old woman has been referred to a dermatologist for an unusual nodular rash on her shins. She led to the consulting room by a nurse, which is a short walk down the corridor. When she is greeted by the doctor, she is almost too out of breath to respond. Embarrassed, she apologises and says that this has become a significant problem of late, and that she keeps meaning to start exercising again.

 

5. A 25 year old man visits his GP complaining of a persistent productive cough of several months duration. He has noticed that his sputum smells foul and has noticed blood in it once or twice. His GP knows him well as he visits regularly to monitor his Cystic Fibrosis.
