1. A 20 year old boy who is type I diabetic was admitted to A&E with a history of fever, vomiting, seizures, and altered consciousness for last 5 days; a non-healing ulcer on the dorsum of left great toe had been present for five months. A course of antibiotics was administered empirically. However, the lesion did not heal and became ulcerated. CSF culture showed Gram-positive spherical budding yeast cells.

 

 

 

2. A 43 year old man had life-threatening pneumococcal pneumonia requiring 3 weeks intensive care and a tracheostomy. About 12 months later, he re-presented with recurrent chest infections and a fungal ball was seen in the area of cavitation on CT scan.

 

3. A known HIV positive 38 year old man presented to his GP with a painful lesion in his tongue and often bleeds if rubbed or scraped. On examination the lesion was tender and had cottage cheese-like appearance.  

 

4. A 5 year-old child was brought to her GP by her mother as she complained of itching of the scalp and bald patch.  

 

5. A traveller returned from East Africa where he visited lots of caves with flu like symptoms. Diagnosis was made by sputum culture.
