1. A 24 year old lawyer is sent to the medical admissions unit by her GP after a 5 day history of malaise and a dry cough. She complains of painful mouth ulcers, and on examination there are multiple lesions over her tongue and lips. Her chest x-ray shows diffuse, subtle shadowing.

2. A 29 year old veterinary nurse presents with a high fever and aching joints. She has been coughing a lot recently and reports two nosebleeds within the last 48 hours. On examination there is some splenomegaly and you notice a pink rash on her face. Her white cell count is 4 x 10^9 / L

3. A 63 year old man was admitted with a severe exacerbation of long-standing COPD. He was transferred to ITU and ventilated. He has not improved on Augmentin. Suction is used to clear copious green secretions.

4. A 35 year old businessman presents to A&E with a 2 day history of coughing and production of a dark red sputum. His chest x-ray shows a patch of consolidation in the right lower zone. WCC is 16 x 10^9 / L

5. A 5 year old girl started school 2 weeks ago. She has had a cough and cold for 1 week with a slight fever. Her mother has brought her to A&E because she is now complaining of ear pain and her cough has not resolved. On examination her ear drum is erythematous and inflamed and there is wheeze in both lung fields. WCC is 10 x 10^9 / L.
