1. A 32 year-old motorcyclist falls off his bike as he swerves avoid hitting a car. He sustains an open complicated fracture of the tibia and on arrival to hospital requires a blood transfusion. A week later on the ward he starts to feel feverish. Blood tests show a fall in hematocrit and rise in LDH.   

 

2. During repair of an abdominal aortic aneurysm, a 45 year-old loses a significant amount of blood. The surgeon requests a unit of type and cross-matched blood to be transfused. Fifteen minutes after this is started, the anesthesiologist notices a significant fall in the patientŐs blood pressure.  

 

3. A 5 year-old, recently diagnosed with acute lymphoblastic leukemia, was given a blood transfusion to correct his severe anemia before initiating chemotherapy. A week after the transfusion his mother brings him into the emergency room – he has had bloody diarrhea, a fever and a blistering rash is evident on his palms.

 

4. Following a complicated delivery, a 30 year-old lady requires a blood transfusion. She develops a fever while recovering on the ward a few hours later. She complains of severe back pain and a headache. Her blood results donŐt show evidence of a hemolytic process. The registrar learns that she has received transfusions during delivery of her other two children.   

 

5.  A 15 year-old patient with a history of asthma is in the middle of receiving a blood transfusion when he starts to experience severe generalized itching. Within moments he starts wheezing and loses consciousness. The transfusion is stopped and he requires resuscitation.
