For each
scenario below, choose the most appropriate answer from the list above. Each
option may be used once, more than once or not at all.
1. A 65 year old alcoholic man presents with progressive dysphagia and retrosternal chest
pain. He has been losing weight which he attributes to his difficulty eating
and drinking. He was diagnosed with BarrettŐs oesophagus 10 years ago but has
not been attending follow up appointments.
2. A 52 year old man presents with an episode of haematemesis.
He has a history of epigastric pain, which is worse
at night and relieved on eating. CLO test was positive.
3. A 50 year old man presents with a long history of epigastric
discomfort related to meals. Endoscopy reveals reddened mucosa with no evidence
of ulceration. Biopsy shows infiltrate of lymphocytes, plasma cells and neutrophils in the mucosa. Staining reveals numerous H. pylori organisms.
4. A 34 year old woman presents with a three month history of tiredness,
malaise and weight loss. She has also been experiencing diarrhoea and some
abdominal discomfort. Duodenal biopsy showed partial villous atrophy with an
increase in intraepithelial cell lymphocytes.
5. A 65 year old woman with a history of thyroid disease presents with a
long history of epigastric pain. Biopsy reveals
glandular atrophy of the gastric mucosa.
